3 men repairing conveyor belt-one man had a crowbar lifting the pulley up while EE had his hand underneath repairing belt-crowbar slipped causing EE's left thumb to be crushed between bearing and adjuster plate.
WORKING AROUND THE PLANT SHOVELING MATERIAL NEST TO PLANT WHEN SUBSTANCE WENT IN EYE PAST - NO PPE - EYE & FACE PROTECTION THEIS ACCIDENT WAS NOT REPORTED UNTIL 4/27/99 BY EE!! ACTING SUPERVIS OR DID NOT FOLLOW COMPANY OR MSHA REGULATIONS FOR REPORTING AN ACCIDENT TO SAFETY SUPERVISOR/HUMAN RESOURCES. FIRED!! (NO LONGER EMPLOYED BY THIS COMPANY.)
EE WAS PULLING ON ELECTRIC MOTOR UP THE PLANT STAIRWAY WHEN HE EXPERIENCED PAIN IN GROIN. RIGHT SIDE/HERNIA THE INJURY DID NOT BOTHER HIM UNTIL 4/22/98 WHEN HERNIA WA DISCOVERED BY PERSONAL DR. HE RETURNED TO WORK 4/2/98BUT WAS PUT ON RESTRICTED WORK WHEN DISCOVERED ON 4/28/98
