Loading Concrete Blocks onto Lowboy Trailer. Loader was backing away when chain hooked on block, pulling it off trailer. Injured was struck by block in the left foot. Foot was crushed and later amputated.

HE WAS DRIVING WITH THE BOX OF THE MOXY UP AFTER DUMPING A LOAD OF BANK RUN & WENT UNDER THE BOOM OF A CRANE INSTEAD OF GOING AROUND IT. THE BOX GOT CAUGHT ON THE BOOM AND LIFTED THE MOXY UP & TIPPED IT ON IT'S SIDE, BUT HE HAD HIS SEAT BELT ON.
