Two rocks got wedged in the feeder. Employee climbed into the feeder where he slipped and landed on his wrist against rocks.
As employee was grinding an air arced area a piece of debris went under his safety glasses and into his eye.
While descending on the steps of the tool trailer, employee stepped off the last step and on to a rock, resulting in rolling his ankle.
