September 21, 1999 WI · Metal/Non-Metal scalper-screen operator SLIP OR FALL OF PERSON

EMPLOYEE WAS UP ON PEP SCREEN ATTACHING A CHUTE.HE HAD ONE SIDE BOLTED AND WAS WORKING ON THE OTHER, WHEN THE FIRST BOLT BROKE AND THE CHUTE FELL TO THE GROUND. HE FELL TO THE GROUND AND LANDE D ON TOP OF THE CHUTE.
