Employee was attempting to strike the overflow barrel of the cone with a mall to break up ice that had accumulated over the weekend. When he swung the mall backward, his foot slipped, he lost his balance and fell over the handrail to the ground resulting in his injury.

While picking and throwing wooden blocks into the bucket of the skid loader the operator tipped the bucket back just as he threw a block causing the injury.
