Milestone Materials Div of Mathy Construction Co · Struck by... (Not Elsewhere Classified)

Employee was assisting to remove cutting edge bolts from a loader bucket that was properly blocked & locked out. Employee failed to use PPE. Since injury we have retrained employee as well as gave additional task training. Employee got EE's finger cut which required five stitches. No long term impairment.

Employee drove a loader in 3rd gear into the sand wall to scoop up a bucket full of material for the plant and felt a sharp pain in her back.
