Employee was using a two pound hammer to hit a frozen/stuck pin on a skid loader so the attachment could be changed. When hitting the pin some debris entered employee's eye. Employee had to have debris removed from eye. Safety glasses were worn.

Employee was dismounting a Caterpillar 769C haul truck when his left foot became wedged between the bottom of the bumper and the factory mounted hanging bumper step. Right foot slipped off the bottom step. This action caused the employee to lose his balance and 3 points of contact and he then fell to the ground. The doctor immobilized his leg and an MRI will be done on 10-4-13.
Employee was clearing out material by hand that had plugged the crusher. He got his finger mashed between the rock he was clearing and steel plate. X-Rays showed no broken bones. He was prescribed an anti-biotic and a pain reliever for the injury.

Employee was climbing out of a 18 ton stockpile truck when his foot slipped off the bottom step. His body shifted to the right and his left knee struck the door brace causing a knee strain and contusion. He was fitted with a moveable knee brace and was prescribed anti-inflammatories for swelling

Employee was unlocking a 5th wheel on a lowboy tractor and trailer. It was stuck and had to pulled hard to unlock. The employee injured the rotator cuff in his left shoulder that will require surgery to repair. Surgery is scheduled for 6-24-09.

A new roll of belting was elevated off the ground with the boom of a service truck. There was a drill steel put through the center of the belting. The load shifted and the employee grabbed one end of the drill steel. His finger was mashed between the drill steel and the bumper of the service truck. He received stitches and broke the tip of his finger.
