EE WAS CHANGING OIL OF MACK STOCKPILE TRUCK. WHEN REPLACING THE DRAIN PLUG THE EE BRUSHED UP AGAINST THE OIL PAN AND SOME ACCUMULATED DIRT FELL BEHIND HIS SAFETY GLASSES INTO HIS LEFT EYE. EE ORIGINALLY FELT LITTLE DISCOMFORT BUT AFTER 3 DAYS HE NEEDED TO SEE A DR.

SEVEN EMPLOYEES WERE WO RKING ON THIS SCREENING PLANT AND NONE HAD LOCKED OR TAGGED IT OUT. AS A RESULT ALL SEVEN EMPLOYEES AND THE PLANT MGR WERE SUSPENDED FOR THREE DAYS. EMPLOYEE SUFFERED A BRUISED LEFT ARM & HIP

ON 7-21-86 EMPL2YEE WAS DRILLING ON A LEDGE FROM BACK TO FRONT APPROX. 10 HOLES IN THE ROW. THE LAST ONE AT THE FACE. EMPLOYEE STARTED DRILLING HIS HOLE. WHEN THE DRILL WAS FINISHING. EMPLOYEE WALKED AROUND 7O SHUT IT OFF AS HE WALKED TOWARDS THE FACE HE COULD NOT SEE WELL BECAUSE OF THE DUST AND WALKED OFF LEDGE, BREAKING LEFT WRIST, CUTTING LEFT CHEEK, CUTTING RIGHT ARM.
