EMPLOYEE WAS TIGHTENING WIPER ON DISCHARGE CONVEYOR. HE HAD CLIMBED ON TEH BELT WHEN THE CONVEYOR WAS TURNED ON. HE WAS THROWN 8-10 FEET TO PIGGY BACK ON NEXT CONVEYOR. SEVEN EMPLOYEES WERE WO RKING ON THIS SCREENING PLANT AND NONE HAD LOCKED OR TAGGED IT OUT. AS A RESULT ALL SEVEN EMPLOYEES AND THE PLANT MGR WERE SUSPENDED FOR THREE DAYS. EMPLOYEE SUFFERED A BRUISED LEFT ARM & HIP
EE WAS CHANGING A TIRE ON A DUMP TRUCK USING A 3/4" DRIVE BREAKER BAR AND AN EXTENSION BAR (CHEATER). THE EE JUMPED ON THE ENTENSION WHICH SLIPPED OFF THE WHEEL NUT AND HIT HIS RIGHT ANKLE.

EMPLOYEE WALKED AROUND 7O SHUT IT OFF AS HE WALKED TOWARDS THE FACE HE COULD NOT SEE WELL BECAUSE OF THE DUST AND WALKED OFF LEDGE, BREAKING LEFT WRIST, CUTTING LEFT CHEEK, CUTTING RIGHT ARM.
