Employee pinched EE's finger between two metal stands while moving the stands.

Employee had a small piece of metal blow into their eye and become embedded.

Employee was walking around truck when left foot broke through the ice. Employee's right foot slipped on the ice and employee fell to the ground breaking lower leg.

Throwing wooden blocks into loader bucket, felt sharp pain in lower back

Pushing air to air line back onto pipe, bracing against battery box of loader with side of body. Pushed hard enough against battery box to crack a rib.

Employee was pulling a rope attached to a conveyor belt by a clamp, clamp came free and hit employee on the right hand.

Lifting/throwing stone, felt tinge in lower back

As employee scraped the floor of the quarry with the loader, the bucket dug deep into the floor resulting in the loader stopping suddenly and he struck the windshield.

While putting together and taking apart water pipe for the wash plant employee slipped causing a lower hip strain.

EE SLIPPED WHEN STEPPING OFF THE SKID STEER BUCKET AND FELL ON HIS RIGHT SHOULDER. APPARENTLY OIL AND COLD MIX ON BOOTS.

WHILE CHANGING A STEEL, EE WAS THROWN BACK [A CHAIN BROKE]. THE WEIGHT OF THE STEEL PUSHED HIM BACK & TWISTED & BENT HIS BACK TO THE RIGHT.

EMPLOYEE STEPPED OVER A WATERPIPE AND BOTH FEET SLIPPE DOUT ON THE WET, ICY SURFACE. HE LANDED ON HIS ELBOW WHICH JARRED HIS SHOULDER.
