THE DOOR WENT UP AGAINST A 4K LB BREAK REPAIR MACHINE WHEN I NAME CLOSED THE DOOR THE BREAK MACHINE FELL DOWN ON HIS LEFT FOOT THE ACCIDENT HAPPEN ON THE 6-1-84 ARE DAY OFF WAS 6-2-84 HE TRIED WORKING ON IT 6-3-84 HOLD BACK UP AND WAS OFF WORK 6-4-84.
I NAME UNLOADING A 5 TON TRUCK THE DRILL STEAL WAS TOO HEAVY HE WAS IN A BAD POSTION AND STRAIN HIS LOWER BACK WESDAY 23 THURSDAY 24 HE STAYED IN BED HIS BACK BECAME MOVE PAINFUL AND VERY HAR D TO MOVE THE EMT WAS NOTATIDE THAT NIGHT AFTER EXAMING DR RECOMANDEATION WAS TO FLY HIM TO ANCHORAGE TO HOSHITLE.
