AT SOME TIME DU9ING THE FORENOON I NAME GOT A PIECE OF FOREIGN MATTER IN HIS LEFT EYE. HOW AND WHAT IS UNKNOWN THE EYE BECAME BOTHER SOME DURING THE AFTERNOON AND AFTER WORK HE WENT TO THE LOC AL DR AND HAD I7 REMOVED. WHAT THE MATERIAL WAS NOT REPORTED TO HIS EMPLOYER.

WHILE HELPING A WELDER IT APPEARS THAT SOME FLUX FLEW OUT AND BECAME LODGED IN HIS LEFT EYE. HE DID NOT REALIZE THIS UNTIL THAT NIGHT AND WENT TO THE DOCTOR THE FOLLOWING MORNING.
EMPLOYEE WAS CLIMBING INTO THE CAB OF A PIT TRUCK WHEN HE HIT HIS RIGHT HAND ON THE SIDE OF THE TRUCK
