MOVING 50' TRANSFER CONVER WITH A LOADER, BOTTOM OF CONVEYOR RESTING ON LIP OF LOADER BUCKET, EMPLOYEE UNHOOKED CHAINS HOLDING CONVEYOR TO BUCKET WHEN IT STOPPED. CONVEYOR SLIPPED OFF OF BUCKE T AND FELL TO GROUND LANDING ON EMPLOYEE'S LEFT FOOT FRACTURING IT IN TWO PLACES.
