EE NOTICED A BUILD-UP OF MATERIAL IN A TRANSFER CHUTE. HE BROKE IT LOOSE. THE MATERIAL BLOCKED THE PATH OF THE REST OF THE MATERIAL. HE MOVED TO THE OTHER SIDE OF THE CHUTE TO REMOVE THE BLOCK AGE. IT BROKE LOOSE AND THE MATERIAL PUSHED THE SHOVEL DOWN AND SLAMMED HIS HAND INTO A BRACE OVER HEAD.

THE OPERATOR WAS TOLD TO LOCK OUT THE BELT AND GET A LADDER AND PUT ON AS AFTER BELT. HE DID NOT GET A LADDER OR A SAFETY BELT. AND TRIPPED AND FELL 5' ON SOME ROCK.
I WENT TO LIFT A LARGE ROCK. WHEN LIFTED THE ROCK UNDER IT THAT WAS SUPPORTING IT SLIPPED PINNING AND MASHING FINGER BETWEEN THE TWO ROCKS.
