Two employees were repairing a tear on a conveyor belt. The tear had been cut out and the patch was being cut by the employee using a utility knife. As he was cutting the patch the knife slipped and cut his right calf muscle. The laceration required 5 stitches.

THE LEFT DRIVERS DOO R WAS OPENED AND THE EMPLOYEE WAS GRABBING THE DOOR FRAME FOR BALANCE. WHEN THE RIDE CYLINDER COLLAPSED THE DOOR CLOSED ON HIS FINGER.
