EE was using a jack handle to lower a trailer, when the jack handle sprung back and hit him in his mouth. One tooth was knocked out and one tooth was broken off.
Two employees were repairing a tear on a conveyor belt. The tear had been cut out and the patch was being cut by the employee using a utility knife. As he was cutting the patch the knife slipped and cut his right calf muscle. The laceration required 5 stitches.
EE WAS HAULING SHOT ROCK FROM THE PIT TO THE CRUSHER WITH R25 EUCLID HAUL TRUCK. HE WAS BACKING UP TO GET LOADED WHEN HIS FRONT TIRE HIT A ROCK CAUSING THE STEERING WHEEL TO SPIN AND SPRAINED HIS RIGHT ARM.
