Employee was dismounting loader and they slipped on the mud on the ladder. Employee fell and scraped their lower stomach. Received a prescription for antibiotics.

Loosened bolts to drop jack. Jack fell by itself and hit on left shoulder see attached as to why not reported til now (Went to dr 7/24/06 for x ray-negative, mri 7/31/06 with result, physical therapy because he was still having problem with his left shoulder. Therapy start 8/21/06, go back to dr 9/14/06. Accident did not report until dr ordered physical thearpy)
