Person was having pain in left side of neck, arm & leg. Went to doctor was severely dehydrated. Was admitted to hospital. Been working in excessive heat for several weeks.

The wind blew dust and dirt off of the conveyor and got something in EE's eye. Doctor removed particles from upper eye lid.

Loosened bolts to drop jack. Jack fell by itself and hit on left shoulder see attached as to why not reported til now (Went to dr 7/24/06 for x ray-negative, mri 7/31/06 with result, physical therapy because he was still having problem with his left shoulder. Therapy start 8/21/06, go back to dr 9/14/06. Accident did not report until dr ordered physical thearpy)
