Employee was crawling under rock shoot to gain access to rear panel, while climbing out stepped on rock, twisted and fell they twisted their left ankle and needs a boot and restrictions after seeing the doc.

A Renaissance Fair attendee bypassed a fence and berm to climb on the quarry wall and fell 30-35 feet to the ground below. Authorities said alcohol was suspected and they are still investigating. The injured person is not a Bryan Rock employee and no employees or equipment were present.

While performing maintenance and wearing a welding helmet, employee cut the top of his head on a plastic scraper on a screener requiring 8 stitches.

Employee attended our annual safety class in Feb. 07 and took his annual hearing test. We just received the test results and notified the employee of his change. Employee stated there was background noises during his test that may have affected his test results. Employee will be offered a retest if his so desires.

EE AND A COWORKER WERE STANDING ON A TEMPORARY SCAFFOLD ABOVE A JAW CONE CRUSHER. THE TEMPORARY WELD BROKE CAUSING BOTH TO FALL INTO CONE. HE BROKE A BONE IN HIS RIGHT FOOT.
