Bryan Rock Products, Inc. · Struck by falling object

An employee was installing a spray bar on a wash screen. EE dropped the spray bar on EE's right index finger while attempting to bolt down the spray bar. It fractured the tip and caused a cut requiring approximately 6 stitches on EE's right index finger. Two other employees were on the other side of the screener, but neither visually witnessed the accident.

An employee completed repairs on a conveyor belt. When employee released the belt clamp it sprung and hit the end of employee's index finger and fractured the bone at the end of the finger. The employee is back to work with restrictions and there was no lost time to employee as we paid the employee the full wages for the time employee missed going to doctor.

A miner climbed up on a working platform to help assist in a lift that was being performed to remove an electric motor. The chain came unhooked and the miner was either struck in the head by the chain or suffered the head injury falling off the working platform. The miner suffered a cut and concussion.

RETIRED EMPLOYEE ATTENDED OUR ANNUAL SAFETY CLASS IN FEB. 2005 & TOOK HIS ANNUAL HEARING TEST. WE GOT THE TEST RESULTS BACK & NOTIFIED EE OF HIS CHANGES IN HEARING. ACCORDING TO OUR INSTRUCTORS, WE ARE TO FILL OUT THIS REPORT FORM AS WELL.
