Arrowhead Transport Inc. · Fall onto or against objects

Employee was unhooking a chain after moving a piece of equipment and stumbled on a tooth on the bucket on the front end loader, he fell and hit his mouth on the equipment that was being moved. He was taken to ER in Lisbon ND and had 3 stitches and had a little chip on his tooth. He came back to work approx. 1 hour later with consent of the doctor.
