Plant operator noticed the pipeline was plugging up. Operator and other employee went towards the booster to listen for material moving in the pipeline. They could hear material moving and kept walking towards the plant. They found where the plug was and told dredge operator to shut down. The pipe came apart at a rubber joint and the material and water flew out striking operator.

Employee was going to release a come-a-long on the ADT cab to lower it back down when EE slipped off the edge and fell 7' to the ground fracturing EE's left scapula.

A section of pipe was being removed from the plant dredge. The pontoons have a hook for a sheave that holds the electric cable. They had the cable removed and hung 2 sheaves on the pontoon hook. When hitting the pontoon w/ the workboat to separate the pipe and rubber elbow one of the sheaves flew off crushing employee's left thumb while they sat in the workboat.

The truck had a leak at the hydraulic hose beneath the cab. The seat and an access cover were removed to gain access to the hose that ran on top of the transmission. EE was straddling the transmission to replace the base when they slipped and fell on their tailbone. Sustained a fracture to the tailbone.
