Two workers removing a cyclone part off equipment. Equipment part was unexpectedly heavy due to unknown debris hiding inside. An edge of part hit one worker in face. Equipment was on ground. Temperature was 87 deg F with 61% humidity. No work days missed by IP (injured person).

Plant shut down w/dry work area. Work from ladder tied to structure. Worker in FRC, face shield, safety glasses, gloves, and fall arrest gear. Using 4" grinder to cut metal. Grinder kicked back into workers face causing laceration to chin.

Employee was unsafe in the task they were performing, in direct violation of training and the site safety policy. Employee was standing on a railing and fell off of it. Disciplinary action will be commensed upon return to work. Employee has cut to the head & concussion. Staples inserted to be removed on 4/04/2020.
