Superior Silica Sands, LLC · Caught in, under or between running or meshing objects

Employee removed small access cover top a cyclone (auger). The employee was going to attempt to remove sand. When the employee gained entry, they inserted the middle finger and the index finger of the left hand. There was no sand in the cyclone causing their finger to come in contact with the auger.

Employee was placing a counterweight object on a water hose to prevent the hose from "wiping". EE's gloves were wet and EE dropped the object on EE's right hand.

A Customer came on site and was waiting to get loaded and started having chest pains pulled over to the office where we believed he was having a heart attack and got him stable and called 911 where E.M.S was sent and took him the Hospital.

Miner was trying to clear a clod from front of hopper, when his hand slipped off of prybar and cut his forearm on a piece of angle-iron where he was working. The angle-iron is part of the guard on the conveyor.
