THE POWDER CREW WAS IN THE PROCESS OF SETTING OFF A FLOOR SHOT. THE POWDER MAN TOLD THE POWDER MAN HELPER HE WAS GOING TO WALK UP TO A POINT AND YELL BACK AT HIM WHEN TO FIRE THE SHOT. THE POW DER MAN GAVE THE SIGNAL TO SHOOT. A FLY ROCK WAS BLOWN OVER A EST. 200-250 FT BLUFF AND HIT THE POWDER MAN IN THE HEAD.

Mechanic removing bin out of top shelf of service truck tool box. Had both hands on each side of bin while pulling it out. When the back portion of the bin came out it dropped and smashed his left hand pinky finger between the bin and the shelf of the tool box. Received 10 stitches to his left pinky finger.

EE had begun walking from his company service vehicle on ground level within the secondary plant of the surface portion of the mine when he landed on a 1" size rock causing his left knee to twist. He experienced discomfort immediately, went to the company doctor and ultimately was diagnosed with torn cartilage in the knee. He is on light duty until completing surgery.

EE FAILED TO LOWER TRUCK BED AND DROVE INTO THE ENTRANCE OF THE UNDERGROUND TUNNEL. EE HIT HIS HEAD ON THE STEERING WHEEL CAUSING A LACERATION ON HIS FOREHEAD. THIS LACERATION NEEDED 2 STITCHE S.
EE WAS PUSHING BRUSH WITH A DOZER. A TREE LIMB CAME BACK ACROSS THE DOZER BLAD & HIT HIS LEFT HAND & THUMB. MEDICAL TREATMENT WAS GIVEN. X-RAY, NO FRACTURES. THUMB WAS BRUISED & DRAINED BLOOD FROM NAIL.
THE EMPLOYEE WAS OBSERVED EARLIER THAT MORNING HIS LOADER AND HAVING ANY PHYSICAL PROBLEMS NOR DID HE VOICE HIS CONDITION TO ANYONE AT THE QUARRY. HE WAS FOUND BY A CO-WORKER ABOUT 12PM THE C O-WORKER IMMEDIATELY CHECKED HIS VITAL SIGNS, BUT THERE WAS NOTHING, EMS WA NOTIFIED THEY ARRIVED AT THE QUARR SOMETIME AFTER 12PM, EMS THEN NOTIFIED COUNTY CORNER. THE CORONER PROUNCED DEATH
OTHER THAN REGULAR WORK ACTIVITY NO CONTRIBITING INCIDENT COULD BE ASSOCIATED WITH THE RECURRING INJURY. AS A RESULT OF THE RECURRING BACK INJURY CAUSED BY HIS ACCIDENT ON 3-12-92, EMPLOYEE WA S PUT ON LIGHT DUTY BY HIS DOCTOR. ATTACHED IS A COPY OF THE ORIGINAL ACCIDENT REPORT.

EMPL WAS HELPING LOAD POWDER INTO FLOOR SHOT IN PIT QUARRY. HE WAS STANDING ABOUT 4' FROM HIGHWALL ROCK WEIGHING 3/4 LB & MEASURING 4 1/2"X2 1/2" FELL OFF HIGHWALL FROM ABOUT 25' & STRUCK EMPL ON RIGHT ARM.
