Employee was lifting up EE's right leg as EE was about to step over a loader rut at the facility. As EE lifted EE's leg in the air EE felt pain in EE's right hip. Employee went to ER on 1/12 for pain. Employee visited the clinic and was given restrictions on 1/13.

Vulcan Construction Materials, LLC · Struck by falling object

EE had to remove the bolts from the skid plate on haul truck to access the oil valve for the oil change. The manufacturer-provided chains were used to secure the skid plate up without bolts, but the chains failed. The skid plate fell approximately 4" and struck the employee's right hand/wrist. EE sought medical attention the following day and diagnosis was a fractured wrist.

Employee was replacing tail pulley bearing on conveyor. Bearing housing shattered when pried on resulting in a piece of metal flying into employees eye under safety glasses. Employee received minor scratch to left eye.
