Clt was stepping from skid loader bucket to the ground, he caught his foot in a chain twisting his ankle as he fell to the ground.
As dozer crested the top of travel way ramp the center of gravity pulled the front of the dozer down. Employee was thrown forward into the cage of the dozer from the seatbelt not being buckled correctly.
