EE HAD TROUBLE WITH AUTOMOBILE, CAME IN LATE FOR WORK. WHEN HE WENT TO REPLACE HOLD OVER EE, HE PLACED HIM ON A DIFFERENT SIDE OF THE TWIN HEAD BOLTER THAN NORMAL. EE PUSHED THE WRONG LEVER. R ESULTING IN HIS HEAD BEING STRUCK BY THE BOLTER POT & ATRS.
