THE PLANT OPERATOR/LOADER OPERATOR WAS FOUND WITH HER LEFT ARM ENTANGLED IN THE SMOOTH TAIL PULLEY OF THE SCREEN FEED CONVEYOR. A COMPANY EE FOUND THE VICTIM AFTER SHE FAILED TO RETURN TO THE MINE OFFICE AFTER HER WORK DAY.

CREW USED A SIPHON HOSE TO CARRY WINDSHIELD WASHER FLUID FROM A 55 GAL DRUM TO A 325 GAL TANK. FLUID IS MIXED W/WATER FOR DRILLING FLUID. DRUM TO HOSE SLIPPED OUT OF THE 325 GAL TANK. EE GRABB ED HOSE TO SUCK ON HOSE TO START SIPHON, IN THE PROCESS, HE SWALLOWED SOME OF THE WASHER FLUID.
