EMPLOYEE WAS DRIVING LOADER AT MEDIO SITE WHEN HE HIT A POTHOLE WHICH JOLTED THE LOADER AND JARRED LEFT SIDE OF BODY. WENT TO DRS ON 3/1/01. GIVEN PRESCRIPTION FOR PAIN. OFF WORK TILL 3/12/01.
