H & D Coal Co., Inc. · Fall to the walkway or working surface

Conditions were wet and muddy. Injured was attempting to get on swivel truck and right foot slipped while left foot was on non-skid surface. Lost balance, weight shifted, twisting left leg and resulting in fall from truck. Was taken to nearby clinic and preliminary diagnosis is possible torn quad tendon / muscle.
