SUBJECT WAS STANDING ON A BELT CONVEYOR CLEANING AN OVERLOAD OF COAL FROM THE TAILPIECE. AS HE WAS SHOVELING, THE BELT WAS STARTED AT THE BELT DRIVE BY THE BELTMAN. SUJECT TRAVELED APPROX. 60 FEET ALONG THE BELT CONVEYOR AND WAS ABLE TO DISMOUNT THE BELT BY GRASPING A CHAIN TO SUSPEND THE BELT.

A FALL OF SANDSTONE ROOF MATERIAL OCCURRED AT THE APPROX. TIME AND DATE MENTIONED ABOVE. THIS FALL IS LOCATED ON THE RETURN AIR COURSE OF THE 002 MMU, APPROX. 200 FT OUTBY SURVEY STATION 11578 . THE FALL WAS 12 TO 20 FT WIDE, 4 TO 5 FT THICK AND 65 FT LONG, THE AREA WAS SUPPORTED WITH 42 INCH RESIN GROUTED ROOF BOLTS. THIS AREA HAD BEEN DANGERED OFF PRIOR TO THE FALL. THE AREA HAS
