71 Construction · Fall to the walkway or working surface

Employee was standing on a conveyor belt working on another belt (2 independent power shut-offs were switched off). Supervisor instructed an employee, in the presence of the entire crew, to start the belts. Injured employee did not think the belt he was standing on would be started but it was. Employee fell 3 or 4 feet and aggravated a previous back injury.

Employee was using a box cutter to cut rubber matting to be used as a guard on a conveyor belt. The box cutter did not cut fully through the rubber mat and the box cutter slipped, causing the blade to cut through the employee's leather gloves and lacerate the left top of hand.
