Revenue Mine: 2 recorded fatalities Metal/Non-Metal Past POV 2014

Employee was working out of a 45' man lift. EE was bolting up purlins. EE had to move the basket around the column to complete bolt up. When EE moved the basket it lurched to EE's left catching EE's finger between the column and the basket.

While tramming an electric locomotive, out of the mine, dust, dirt or another unknown debris entered the miner's eye. The event resulted in an abrasion to the eye and antibiotics were administered to the eye.

While running a stoper in the Alimak raise, the drill EE was operating hung up in the hole and spun, resulting in a smashed right middle finger.

While scaling rock in a raise from a work platform. A rock roughly 5" to 9" fell and entered the work deck striking employee on the right foot. Raise is on a 72 degree angle and the rock fell from the area they were scaling.

An OSMI foreman was driving an electric locomotive into the mine and upon approaching a run around, the foreman reached down for a bottle of water. At the moment they sat up, the foreman fell from the locomotive and was crushed between the locomotive and rib. The foreman suffered broken ribs and a fractured pelvis.

EE was walking on a muck pile when EE slipped and fell to the same level. The fall resulted in an over extension of EE's arm.

While removing a compressed gas cylinder from a storage rack, the cylinder slipped and smashed an employee's right pinky finger between the cylinder cap and cylinder rack. The event resulted in three stitches to the finger nail bed.

EE was standing at the bottom of an unoccupied manway preparing to survey the area. As EE looked up, EE was unable to shine the light upward far enough to see the area. EE removed hard hat and was holding it in EE's hand while shining the light upward when a rock struck EE on the side of the head.

EE was blow piping down the ore chute. EE was wearing fall protection at this time. EE finished blow piping, sat it down on the walk way, took fall harness off and took a step forward. A metal wire poked EE in the face, EE was startled jumped back. EE fell down the ore chute, 30 feet below and landed in the 7 ton young buggy.

An employee was operating a Broderson IC-80 crane attempting to lift part of a crusher assembly when the crane tipped over landing on top of a haul truck box. The operator then unbuckled the seat belt to exit the crane and injured right shoulder.

Employee and partner were installing pipe for utilities when the partner slipped and dropped the pipe which caused the employee to drop their end of the pipe, dropping it on their foot.

While moving cabinet down stairs, one miner missed bottom step injured miner adjusted footing missed step and fractured ankle.

The Miner tried to avoid water coming out of the back. His ROPS struck the rib kicking a rock loose that struck his right forearm.

Miner was taking an air spud off of a water pump with a large pipe wrench. He thought the spud was tight but it was loose. When he pried down on it forcefully his hand struck the metal table breaking his hand.

The miner was attempting to board an electric Loci that he had parked under an 8" air line. When he stepped on the wet step his foot slipped causing him to hit the right side of his head against the air line. He did not report this till the end of shift. He declined first aid or medical attention.

Employee was working at the face, when he turned and stepped on a rock, rolling his left ankle. Employee was seen at ER and released to light duty. Further evaluation revealed the ankle was broken.

Employee was riding into the mine on a rail bike when he encountered an 8-ton battery locomotive. He exited the bike and was trying to get it off the track when the locomotive struck the bike, propelling a piece of the bike into his leg, bruising it.

Employee believes he inhaled an unknown substance - possibly drill oil mist. Developed respiratory issues.

Employee was shoveling out of ball mill feed and heard felt a strain in his shoulder. Employee was evaluated at medical center and returned to duty next shift with restrictions. No lost time.

Employee slipped on icy switch and tore calf muscle.

Employee was working near exhaust of boiler. After work had symptoms of CO poisoning, so went to hospital.

Employee was riding on rear of pulled train cars. Lifted head and back of mine struck his hardhat, knocking him off onto rail. Taken to hospital for evaluation - did not see any injuries - released. Possible minor concussion.

Employee says foot injured several days ago - does not know what happened - was shoveling - noticed foot sore at home. Did not report immediately. Company is contesting this being a work-related incident

Vibration from blasting in an adjacent area caused rock to fall from rib, an approximately 20-30 lb rock struck employee in the back of the neck. Evaluation revealed only deep bruising. Later evaluation revealed fractured elbow

EE was shotcreting mine back - shotcrete hose plugged then became unplugged and sprayed shotcrete up into face - went under safety glasses. Medical care involved flushing eyes and prescription - no lost time.

Employee stated that he had hurt back previous week - thinks handling 4 x 8 panels of wire mesh. Company is contesting the incident as not work related.

Employee noticed something in his eye after returning home from shift. Does not know when exactly it occurred or what he was doing at the time. Reported two days after it happened and then had a speck removed by doctor four days after incident. Doctor prescribed antibiotic ointment.

Moving 12 x 12 timber from one mine car to another, material sprayed into the eyes of the employee. The employee was not wearing eye protection and received the injury.

Two employees were removing a pump from a sump. When removing the pump, one employee felt a pull in his groin and experienced pain.

Pulling hose out of mud and feet slipped, causing a pain in the lower back.

Walking from portal to mine office. Twisted knee while crossing mine track. Heard pop. Went to dr. for evaluation and prescription issued.

Mill operator received flash burns to eyes from welding operations. Believed to be from reflectance

Walking back to loader when slipped on icy ground and landed on back.

1/22/14 received medical documentation stating employee is unable to work due to mental health.

The injured miner was drilling into the back with a jackleg for rock bolting. While drilling, the steel shanked and forced the miner into the rib. The jackleg then hit the miner in the right knee causing the injury. The miner received medical attention.

EE was welding and he flipped his hood up to check the weld when something airborne got into his eye.

Improper lifting of pipe, which employee believed caused hernia. Employee declined to provide information to the company.

The employee was drilling when a rock hit his left hand

Employee was cutting on some conveyor belting, Utility knife slipped and cut the his left hand thru his glove.

Incident found in routine audit - Employee was holding the alimak rail so it could be bolted together. A rock fell out of the back striking him behind his leg.

Received written statement on 3/3/14 - Lifted rail while walking and hurt back.

Incident found in routine audit -While the employee was mucking on the left rib, the bucket caught causing it to tip over and pinning employees wrist against the rib.

Hammering overhead, when some debris fell into the left eye.

A flat car was being hooked up when the employees right hand got pinched between the motor and flat car.

The employee was knocking shotcrete loose from a bag and a piece of shotcrete popped into his eye.

Two men were being lifted up in an enclosed work platform with a telehandler to perform task on structural steel frame. The work platform was passing by frame work and injured worker had right arm out.

The employee was swinging a slug hammer to drive spikes in the rail and felt a pop in the right elbow.

The employee was moving supplies and injured his right ankle.

Incident found in routine audit - 1/16/2013 Employer was notified of incident that occurred on 10/28/2012. No medical treatment was given on 10/28/2013 but on 12/1/2012. Employee caught his boot on a loop if blasting cap wire. He fell forward with his full weight landing on his right arm and knee.

Jack leg operator using G.D.-83 drill to pin a heel plate on the welded wire, While the miner helper placed the nut on bolt. After applying the nut the operator lost anchor point on the drill and the drill steel came around and struck the miner helper in the ear & neck.

While installing resin cartridges in a bore hole, a slab of rock came loose and struck the employee in the nose. The miners were working from a supported area trying to tie the mats and bolts into the face and back.

EMPLOYEE & PARTNER WERE MOVING A 4 X 8 STEEL PLATE PARTNER DROPPED ONE END CAUSING JARRING OF EMPLOYEE RIGHT ELBOW.

EE WAS LOADING HOLES WHEN SLAB CAME FROM FACE & HIT EE ON HEAD & RIGHT SHOULDER, LACERATION OF BACK OF HEAD.

EE WAS STOPPING & SLIPPED ON ROCK TWISTED KNEE. EE RECEIVED A MEDICAL MENISCUS TEAR OF THE LEFT KNEE.

EE WAS HELPING PARTNER COLLAR HOLE. IR300 DRILL MACHINE & STEEL HIT EES LEFT TRICEP WHEN DRILL STEEL SHANKED. EE RECEIVED A TRACT INJURY TO THE LEFT AUXILARY NERVE.

WORKING IN DRIFT HANGING AIR HOSE WALKED INTO LOW BACK-JAMMED NECK. A FEW MINUTES LATER-HIT HEAD AGAIN & HAD SHARP PAIN & STIFFNESS IN NECK. RESULT WAS TRAUMATRC CERVICAL APOPHYSTIS.

ROCK BOLTING STEPPED BACK ONTO A ROCK BOLT & PLATE BOLT FLIPPED UP & HIT MINER ON RIGTH SIDE OF FACE.

EMPLOYEE WAS UNLOADING STEEL FROM SKIP & INJURED BACK

EMP WAS PULLING CABLE FROM A SHUSHER AND SPRAINED BACK

EMPLOYEE WAS HELPING MINERS SET-UP IN FACE AND WHILE DRAGGING HOSES ROCK FELL FROM BACK AND HIT EMPLOYEE ON RIGHT HAND.

PARTNER WAS BARRING DOWN SLAB. WHEN SLAB CAME DOWN PARTNER JUMPED BACK AND HIT EMPLOYEE ON SIDE OF HEAD WITH END OF SCALING BAR.

SNOWSLIDE RAN OVER MINE PORTAL BLOCKING ENTRANCE TO MINE. MINERS DUG OUT OF SNOW AT 9:50 PM

PULLING JACKLEG OUT OF HOLE-SLIPPED ON TRACK AND CAUGHT LEFT HAND BETWEEN DRILL AND LEG. CONTUSION TO LEFT HAND.

HOIST MOTOR BROKE DOWN AND MAN HAD TO BE BROUGHT OUT OF SHAFT BY CLIMBING LADDERS IN MANWAY. MOTOR WAS REPAIRED OVER WEEKEND AND PUT BACK INTO SERVICE ON MONDAY 2/7/83 MICK DENNEHEY WAS CALLED AT 8:15 PM ON 2-4-83 TO NOTIVY MSHA OF ACCIDENT

CLEANING CLEVIS PIN WITH WIRE B RUSH ON GRINDER - METAL BURR GOT PAST SAFETY GLASSES AND WAS IMBEDDED IN LEFT EYE, BURR WAS REMOVED BY DOCTOR ON 2-5-83
