An unintentional roof fall was found in the intersection of the No.5 entry 2t crosscut 11. The roof fall is estimated to be 20'w x 15' long x5' high. There were no property damage or injuries.

An unintentional roof f all occurred in the intersection if the #8 entry and the 2nd c.c. The fall was approx. 30' L. X 18' W. x 8-9' H. The fall was discovered during the preshift exam. The date and time of the fall could not be determined.There were no injuries and the fall did not impact ventilation or travel of person into or out of the mine.

An unintentional roof fall was found in the intersection of the No.# entry at c.c. 16. The roof fall is est. to be 30' W. X 20' L. X 6' H. There was no property damage or injuries. The fall may be an extension of an existing fall that was found in the xcut to the left of the intersection.

Employee was pulling air hose and a clamp on the hose cut the 5th digit on the left hand. (Medical only).

An unintentional roof fall was found during the preshift exam in the intersection of the 7th cross cut and No. 14 entry. The fall was approx 18' wide x 10' high x 20' + long (exact dimensions not known until clean up). MSHA was notified and the approaches timbered off. No injuries, property damage, or limitations to travel resulted.

Employee was attempting to "walk" an ATRS jack from a location where it was removed to the coal rib. The employee became unbalanced and the jack caused him to fall. The jack fell across his leg resulting in contusions to the employee's upper left leg and thigh. (medical only).
