SEWARD SAND & GRAVEL INC. · Fall from machine

Sunny, cold day, light winds, ambient temperature approximately 15 degrees F. Subject slipped and fell while entering a truck. Surface of the ground was packed snow with small gravel sprinkled over it to aid traction. Damage according to physician was fractures to ribs and vertebrae.
