Miner was attempting to clear an obstructed troughing roller with a small hammer. The miner was standing on the cross support of the screen deck #1, clearing a roller on the 1st under screen belt. Miner slipped and EE's right arm fell into the back side of the guarding, EE's right hand came in contact with the belting and roller resulting in a cut to EE's right middle finger.

Operator with fully loaded haul truck traveled down from upper loading area, along approximately a 1,000' long haul road with an approximate 15% grade. Operator noticed a situation with the brake system on truck. As EE approached lower storage area EE was unable to stop. Operator diverted truck against stored material, contacting stored boulders, overturning dump bed and stopping.

Loader operator slipped while descending equipment ladder, spraining right foot.
While setting steel deck plate into position, contractor lost control of plate, allowing plate to shift/slide into ladder. This sudden movement dislodged worker off ladder, individual fell approx. 6'8" to ground.

While changing out a damaged gear box, cleaning the shaft with a grinder, grinder bounced off the head pulley, kicking back and making contact with his left index finger. The incident opened a one inch gash which required 4 stitches to close.
Mechanic, after repairing /replacing screens on screen deck at old plant, had his finger (right pinky) push thru screen. Finger hit screen deck assembly and was broken. Emergency room visit was required where finger was isolated & splinted. On 7-20 surgery was performed on finger.

CHECKING CRUSHER SLIPPED ON A RUNG ON THE LADDER AND FELL BRUISED LEFT THIGH CHEEK AND EAR.
DRIVING WATER TURCK AROUND PIT AND HIT A HOLE. THE TRUCK WAS FULL OF WATER AND IT SHOOK VERY VIGOROUSLY. EVEN THOUGH THE DRIVER DID HAVE HIS SEATBELT ON, HE WAS STILL THROWN ABOUT THE CAB.
WAS LOOKING UNDER TRAILER TO CLEAN OFF SOME EXCESS MATERIAL. LOOKED UP AND HEAD HIT A PIECE OF EXPOSED STEEL CAUSING A LACERATION TO THE FOREHEAD.
CHANGING FILTERS ON D9 SLIPPED OFF D9 AND FELL TO GROUND.
