United Metro Materials Inc · Flash burns (electric)

EE WAS OPERATING CRUSHING & SCREENING PLANT. ENTERED CONTROL ROOM (ELECRICIAL) AND NOTICED 2 BREAKERS WERE TRIPPED. AFTER RESETTING BREAKER FAILED, EE OPENED BREAKER LID. UPON INSPECTION NOTIC ED 2 LOGS WERE LOOSE. HE PLACED BREAKER HANDLE IN OFF POSITION. ATTEMPTED TO TIGHTEN LUGS W/SCREWDRIVER. SCREWDRIVER MADE CONTACT WITH METAL TRIP MECHANISM CAUSING PHASE TO GROUND CONNECTION
