HE WAS CHECKING THE SCREENS ON THE PLANT. WHEN HE WALKED DOWN THE STAIRS HE SLIPPED OFF THE LAS STEP CAUSING HIM TO FALL AND INJURE HIS BACK.

HE WAS HOLDING A WRENCH AND ANOTHER EMPLOYEE WAS HITTING IT WITH HAMMER. DURING A SWING THE HEAD OF THE HAMMER CAME OFF AND STRUCK HIM ON THE HEAD. AFTER HE GOT OFF WORK HE DECIDED TO GO TO TH E HOSPITAL. HE TOLD NO ONE OF THIS. CO. WAS NOTIFIED OF HIS MEDICAL CARE ON 3-23-98.
