Alpine Silica, LLC · Fall to the walkway or working surface

An employee was using a break-over binder and chain to secure poly pipe to a float, the chain slipped, causing employee to fall awkwardly, (same Level) breaking EE's lower left leg.

Performance Proppants, LLC · Fall from machine

One of our safety rack techs fell off a tanker truck. The driver of the truck drove off before our EE could get off the top. EE was taken to the hospital for evaluation but had no major injuries. EE is very sore and bruised up.
