EE was using a 9"grinder to remove some old welds from the dryer. EE was tied off and sitting on the dryer while grinding. EE started to slip so EE reached back to grab the safety line to regain balance causing the grinder to strike EE's knee. EE received 10 stitches to the right knee and was given a post-accident drug screen.

Employee was using a knife to cut and repair a conveyor belt when the knife slipped and cut EE on the forearm. Brought employee to medical facility to get 7 stitches in forearm. EE was released back to full duty the same day.

Two miners were replacing a pipe on top of a conveyor tunnel. The pipe was secured to the backhoe bucket with a chain. When the backhoe driver lowered the pipe into position, the bucket allegedly struck the miner on EE's upper back.

EE was driving boat up to the pipeline to get a rope and tried to get the motor to go in neutral. The motor was hung in gear. He did not have time to kill the motor so he jumped on the pipeline as the boat kept going. He has bruises/contusion on his right side. Xray shows bruised ribs.

Employee was standing on the platform of 966F loader and was lifting the seat of the loader. The seat got hung up in the door of the equipment. He then attempted to remove the seat from being jammed in the door and he strained his neck.

Employee was working on shaker at sandplant installing vbelts. Shaker was unlocked to run excess material off of it to see if belts were lined up. He told co-worker to turn shaker off & he did. The counterweight was still moving as ee was proceeding to put guard back in place. The counterweight struck guard causing guard to strike him cutting his right eyebrow.

EE was looking at a coupling using the boat. He went under the pipeline and while going underneath the pipeline, his left leg became wedged in between the boat and pipeline causing swelling around the knee.

THREE EMPLOYEES WERE DEWINTERIZING THE PLANT USING THE JON BOAT TO DO SO. THE BOAT WAS CRANKED; IT WAS IN GEAR WITH THE THROTTLE WIDE OPEN. THE BOAT LUNGED FORWARD, EJECTING TWO EMPLOYEES INTO THE WATER. THE BOAT STRUCK THE STRIPPING BARGE &SMASHED INJURED'S THUMB OFF. THE THIRD EMPLOYEE PULLED BOTH VICTIMS OUT OF THE WATER. INJURED ALSO HAD A HAIRLINE FRACTURE TO THE LEFT WRIST.

THE COILS ON THE HEATER IGNITED THE OIL CAUSING A FLASH FIRE. THE OPERATOR TURNED THE HYD'S OFF & EXTINGUISHED THE FIRE. HIS SKIN ON RT HAND, RT. SIDE OF FACE (INCLUDING EAR) WAS SINGED. THE DREDGE SUFFERED MILD DAMAGE, MOSTLY PLAST
