Pending Investigation - Victim was leveling material in a trench made for laying electrical conduit, manager was operating front-end loader above. The front-end loader was being used to back fill the trench when the bank collapsed causing the front-end loader to slide into the trench. The victim was fatally injured when victim was pinned between loader and jersey block.

Injured Employee was utilizing a torch to cut off two track bolts on an Excavator. As the employee finished the last bolt there was a spark causing them to pull the torch back. The torch then made contact with the right hand and burnt through the glove and burning the backside of their right hand causing a 2nd Degree Burn.

Injured employee is allegedly claiming a 8 inch rock fell from an area overhead at the primary crusher frame. Striking him in the right shoulder. The primary crusher was not operating during this clean out process.

Injured employee and another employee had set the crusher ring and were unhooking the crusher ring from the crane. The shackles and straps were laid on side of crusher frame the crane operator started lifting the hoist of the crane and the shackles came off the side of crusher frame and hit injured employee in the mouth, chipping two teeth.

The injured employee was operating a haul truck in the pit. As he descended towards the bottom of the ramp, he lost control of the truck and traveled off the roadway striking his forehead against the windshield.
Injured employee was drilling a hole in the junction box using a hole saw. When the drill bit punched through, the hole saw teeth grabbed the box and the drill slipped out of injured employees hand and hitting him in the mouth. Resulting in stitches.
