Employee was climbing down the front end loader ladder, as EE stepped off the bottom rung, EE slipped and fell.
Employee was in the process of fueling a diesel powered water pump. As they walked to the pump in the rain they slipped on a muddy surface and fell. The employee fractured their right leg during the fall.

Employee operating the excavator, traveled on the south side of the conveyor, parallel to the highwall. Holding 6-8' back from the highwall as they traveled, the material gave way underneath the excavator, tipping over the machine. The employee sustained a fractured rib and occipital condyle fracture. A seatbelt was worn.
2021 · 2 incidents
Employee was pulling a chain across a belt conveyor and the hook flipped up and hit EE in the mouth. The employee did not want treatment on the day it occured. Was then evaluated on 05/18/21 by a dentist and determined teeth would need removed and repaired.
Employee was removing a rock (approx. 25lbs.) from the screen deck. When we tossed the rock, employee felt a pain in back. Employee was seen at the clinic given ice and ibuprofen. After a week the pain had not resolved and was prescribed physical therapy, making it recordable on 05/07/2021.

Employee was conducting overhead welding activities when a piece of metal got into EE's eye. EE did not report it right away. The metal was removed by an eye dr. on 11/20 and was given a prescription.

EE was removing a fork from a fork lift attachment on a front end loader. The fork pivoted, striking the EE in the face breaking their jaw and knocking out a tooth.

The employee was cutting zip ties off of his hard hat to remove the head lamp. The knife slipped and cut the employee in the hand requiring 3 stitches.

Employee called to report that on Friday, 2/14/14, @ 1000 am she was getting a sample for a customer using a 5 gallon bucket & when she went to pick up the bucket to place on tailgate of truck she felt a "sharp pain in the front of the right shoulder that shot shooting pains down the arm to the fingers & up the neck".
