While restacking pallets of damaged hydrated lime bags on 09/11/24, the miner felt a strain in EE's back.

EE was climbing down a ladder when they felt something enter their right eye. EE was wearing all required PPE. EE immediately reported the issue and began flushing eye with saline eye wash solution. After a couple of hours of eye washing, Site manager suggested that the EE go to the clinic to get checked out. EE was prescribed Prednisolone eye wash solution.

Employee was cleaning at end of shift. EE was sweeping up Hydrated lime with a squeegee and a dust bin when lime from the pile became airborne due to the dust bin adjusting quickly and a small amount of dust got into the employee's eyes. The employee did not report any injury until Sunday 06/25/2017 @ 10:50 am. Employee was instructed to visit a clinic and then file a report.

A small amount lime dust fell from buildup on an overhead guard and entered the employee's eye even though the employee was wearing eye and head protection. (High-viz type 1 Hardhat, Safety glasses with side-shields and foam guard inserts on the lens.) Employee was seen by an ophthalmologist and prescribed hydrocodone to relive the pain from the scratches to the cornea.

Employee stepped on a discarded plastic covering while attempting to pull a new roll of 5mil. shrink wrap bags off the pallet they were sitting on; while performing this action, employee slipped on the discarded plastic covering, straining his back.

Working at lime plant, Workers were in all their PPE. Air born lime dust got on workers skin and then into his eye causing irritation. As a preventative measure eye was flushed and he was sent to the clinic to be checked. They flushed they eye some more and sent him to the eye clinic to make sure there was no damage. He was given prescription eye drops and was released.

Employee was pulling plastic bag off of roll, the bracket for the roll broke causing the roll to fall approximately 3 feet down onto the employees left foot.

Employee was packing 35# sacks at the packer station when a bag blew off of the spout. When this happened, employee reached to push the stop button to stop the flow of material and his arm came in contact with the stream hydrated lime causing material to deflect up and around his safety glasses and into his right eye.

EE and a co-worker of maintenance department, were working trying to install a power shaft into a tube on a 41 inch workbench. Co-worker was at one end of the tube, guiding the shaft so it did not score the inside of the tube. EE inadvertently used his middle back, instead of his whole body as one motion, to push the shaft into the tube, thereby causing the injury.
