Miner was shoveling stemming stone into loaded borehole and felt sharp pain in their lower back.

Technician was threading conduit and the pipe being held slipped while tech was trying to clear some debris from threading area. It pinched tech's left index finger, tearing some skin off. Tech fainted, so an ambulance was called and tech was taken to the hospital. Tech received stitches in left index finger and was sent home.

EE BENT OVER AND WHILE LIFTING A PIECE OF STEEL, EE FELT A POP IN HIS BACK. EE REPORTED INJURY ON 4-9-01 HOWEVER DID NOT FEEL HE WANTED TO GO TO DR OR HOSPITAL ON 4-11-00 EE REQUESTED TREATMEN T. CORRENTLY, EE IS OUT OF WORK, AND RECEIVEING PHYSICAL THERAPY.

DRILLING HAMMER WAS IN MOTION TO RETRACT A STEEL FROM THE HOLE. IT WAS NOT IN LINE, SO AS HE ATTEMPTED TO MOVE THE STEEL FROM THE APPROACHING DRILL HAMMER HIS HAND SOMEHOW BECAME ENTANGLED WIT H THE ARRIVAL OF THE HAMMER AND THE CROOKED STEEL.
