Ee reports working in wet ESP at roaster with improper ppe 9/22/25. Alleges mercury got in right eye twice and also exposure through inhalation. Ee diagnosed by DO on 6/8/26 w/ mercury toxicity and cervical radiculitis. Ee filed claim for workers' comp on 6/9/26. NGM has no biological monitoring evidence of exposure leading to removal from work area for this ee.

EE was cleaning a lime discharge chute. The chute went positive, and lime shot into the EE's left eye. Seen at ER, diagnosed with chemical burn to left cornea, transferred to eye specialist. Released to full duty 12/11.

Ee noticed rash on body. It continued to spread. EE was seen at Golden Health, blood draw showed elevated mercury level. Further testing being done to see if exposure caused rash. Reportable for occupational illness.

EE was getting out of light vehicle. EE stepped out and into a parking ditch. The ditch was deep, and EE twisted right ankle. Seen at Golden Health 3/24, released with restrictions. X-rays on 3/25 were positive for a small avulsion fracture.

Ee reported that EE was filling and lifting bags of floc and heard a pop in EE's back. Statements indicated this may be related to non-occupational shoveling. Seen at clinic where EE reported pain. X-rays negative. Ee released to light duty site can accommodate. After investigation, accepted by workers' comp 2/2/23.

Ee was working to unplug sump pump. There was a hole in a boot (PPE not appropriate) and slurry got in the boot, causing a chemical burn. Ee was seen at hospital, diagnosed with a chemical burn to the foot, and released to light duty. Ee was offered duty within restrictions, but refused and took PTO. Ee did not qualify for disability as modified duty was offered.

EE was unloading quick lime, when EE's hose blew EE attepmted to shut off the pup trailer. When EE ran through the lime it hit EE in the back, face, and eyes. EE was not wearing the proper PPE. EE also did not inspect the hose that was used (off EE's truck) properly prior to use.

Maintenance repair in a tank (confined Space). Contractor was working from JLG/Manlift. Equipment malfunction. Equipment stuck in elevated position. At approximately 4:30 pm the employee was removed from tank via overhead opening. No injuries. Employee was task trained to operate equipment. Documentation of a pre-shift equipment inspection.

Injured Employee was making labels for Control Boxes, right next to water Discharge piping routed on the ground in a new construction area. Employee turned and took one step with left foot, right on top of the water lines. Employee's foot slipped causing injury to left ankle and Knee.

Ee was using a one inch impact to loosen a nut. As the nut spun loose, the gun hung up and twisted the impact, pinching the ee's right middle finger between the gun and casing. The ee was diagnosed with a fracture of the fingertip and released to full duty.

Ee was walking through thick slurry material in a containment area. When ee went to pull foot out ee felt a pop in low back. EE was diagnosed with a low back strain. Put off work pending a follow-up on 09/17/18. EE missed one scheduled shift.

EE and another employee were using a "big gun" to hydroblast scale from the walls at the #2 neutralization tank. The big gun was unsecured when pressure was introduced to the system. EE turned to run away from the big gun and ran into a deck mounted motor. This caused deep bruising to EE's right leg.

EE and another ee were using a "big gun" to hydroblast scale from the walls at the # neutralization tank in the Barrick Roaster. The big gun was unsecured when pressure was introduced to the system. EE was pushed down onto the grating ee was standing on by the force of the big gun moving away from the railing.

The freight elevator at the roaster was commanded to travel from the third floor to the twelve floor. The elevator traveled approximately 15 feet in the opposite direction toward the ground floor and stopped in between levels. There was an employee on the elevator during this incident. The employee remained on the elevator for 2.25hrs before being removed.

Removing Liners from Roaster South Mill Feed Chute. Welded picking eye to the liner and used a hoist to remove. Weld on picking eye broke which caused the picking eye to hit injured employees left index finger causing a laceration and a chip fracture.

Ee was rebuilding an air slide. EE was drilling holes through a cloth membrane, and holding the cloth in place with the other hand. The drill broke through the fabric and caught glove on the other hand, pulling it into the drill bit. Seen at the ER, diagnosed with an avulsion, and put off work.

EE was cleaning sump pump, lifting buckets of material, and felt sharp pain in left shoulder. Seen at the clinic and diagnosed with acute strain to lt shoulder. EE seen at clinic on follow-up 10/17/16, placed on restricted duty and prescribed physical therapy.

Ee raising hood on carbon truck, when EE stepped down EE felt pain in EE's lower back. Pain got worse as shift went on, ee reported to EE's supervisor. Worked remainder of EE's shifts and went to Idaho for days off. When EE returned to Nevada on April 6 EE saw doctor and filed a workers' comp claim. EE was placed on restrictions that the site could not accommodate.

The employee's leg was struck by the metal foot pedal of a hydro blast pump resulting in a fracture. The hydro blast pedal housing kicked back and the shift in pressure caused the pedal to pop up.

Ee was climbing an access ladder when he bumped his head on an overhead pipe, jarring him and making him lose his grip on the ladder. He fell backwards onto the concrete. He did not lose consciousness. He was evaluated by a physician and diagnosed with a fractured rib.

While disconnecting a hose from the carbon truck, the hose came loose from fitting and threw the employee off balance. He fell to the ground, fracturing the right wrist. Will have surgery on 04/16/12 for repair. Currently working restricted.

Employees were removing diamond plate covering to get into work area. While removing the final plate, employee lost control of plate and plate fell approximately twelve inches on top of his right foot, resulting in a broken metatarsal bone.
