Employee was operating a Cat D400E articulated truck and hit a hole that jerked his back causing a pulled muscle.

Employee, was leaning over to pick up a water hose out of a highlift bucket when he slipped and fell into the highlift bucket dislocating his right shoulder. EE was transported to General Hospital by Pres. of Ali Co., where he was treated and released.
