OPERATOR HAD FINISHED WIPING OFF FRONT WINDSHIELD & WAS ATTEMPTING TO REENTER CAB OF MACHINE WHEN HE FELL OFF. HE STATED THAT HE DID NOT KNOW WHAT CAUSED HIM TO FALL OR IF HE JUST LOST HIS BAL ANCE. HE FELL TO THE GROUND STRIKING HIS HEAD & BACK ON THE GROUND, RESULTING IN FRACTURED SKULL & 3 SMALL BONES FRACTURED IN BACK.

OPERATOR OF DAVEY DRILL WAS CHANGE DRILL STEELS AND HOISTED THE DRILL STEEL TOO HIGH WHICH CAUSED A GUARD TO BREAK LOOSE FALLING TO GROUND, STRIKING THE OPERATOR ON THE FOOT.

OPERATOR STATES THAT HE WAS ATTEMPTING TO LOOSEN SOME BOLTS ON THE WATER PUMP AT THE REAR ENGINE OF SCRAPER THAT HE WAS OPERATING IN THE PROCESS HE BELIEVES THAT HE STRAINED HIS BACK. HE THEN FINISHED HIS SHIFT AND REPORTED TO WORK ON THE FOLLOWING DAY.; HE WORKED THAT DAY UNTIL 930 AM AT WHICH TIME HE PARKED THE SCRAPER AND WENT TO A CHIROPRATOR.
