Employee was shoveling material from under the conveyor belt and felt slight pain in lower back (failed to report), operating haul truck and went home. It was not until 6-3-2026 (following day) when employee had a hard time getting out of bed for work when it was reported. Resulted in acute lumbar myofascial strain

Employee's right hand was in the line of fire of a block and chain. The block and chain was under tension and shifted which struck employee's index finger. This resulted in a broken finger.

Operator was changing screens on the shaker deck in the morning and felt their back becoming inflamed from their body positioning. Employee was working through the pain operating the haul truck the remainder of the day when they noticed that the pain had become much worse. Was seen at clinic with a lower right back strain and right lumbar radiculopathy.

Employee was performing grinding and welding on shaker on the lower plant repairing holes. Thought had dust in eyes. Later that evening approx. 1030pm the right eye became inflamed and EE went to ER and they found a slab of metal imbedded.

Employee was disassembling a 30 HP water pump for inspection when he took off his gloves to remove the bolts, he then turned the impeller without gloves and lacerated his right index finger. The finger required 9 stitches.
