EE was removing a 6" hose from a pump and felt a pain on their backside/upper leg. EE was discharged from the ER with restrictions to stay off work and follow-up with an orthopedic specialist.

2 employees were lifting a job box when one felt some soreness in back following the lift. EE initial reaction was to ice it but was instructed to go to the doctor the next morning when ee informed supervisor it did not improve.

Snow and Ice was on the step leading into the cab of the vehicle. Employee slipped on the step and fell approximately 3 feet to the ground, injuring his back.

Employee had top cover off motor on vertical turbine pump turning it by hand to check for noise in bearings. His finger caught on a stationary bolt and the inertia of the motor pulled his hand around, dislocating the small finger on his right hand.

Pulling steel rods out of hole - when the drill head was screwing to the rod to tighten he put his hand on the metal stop that the wrench hits against and it smashed his finger between the wrench and the metal shop.

Employee was using water hose off water truck he was operating to clean up spilled material off concrete pad under jaw crusher. After completing the job he layed the hose down to walk back to the truck and tripped over the hose, fracturing a bone in his ankle when he fell to the same level.
