EE states that while using a pocketknife to cut tape, the blade did not lock fully and folded into left hand resulting in a small cut. The EE was taken to urgent care which resulted in the application of 1 stitch.

An employee was blowing dirt off of their clothes and felt as if something blew in their eye. By the start of the shift that afternoon the employee told the foreman about the incident and if it got worse employee was going to the doctor. After their shift, around 2:00AM they went the emergency room to be examined.

Maintenance employee was using an oxygen/acetylene torch cutting the heads off bolts. While cutting the bolts the employee feel heat on EE's lower legs. When EE looked down there was flames on EE's right pants leg. Employee was diagnosed with 2nd degree burns, no lost time or restricted duty.

Employee felt a discomfort in his eyes early 26SEP morning and went to the emergency room. He followed up with an eye doctor on 27SEP morning. Employee was told he had flash burn 26SEP and foreign body was removed from both eyes on 27SEP. Employee and witnesses stated all PPE is worn at all times including the day incident is suspected to have occurred.
Due to sleet and freezing rain, the surface of the lot had quickly become very slick. As our driver walked back to his truck, he slipped on the ice and fell. When he caught himself with his left hand, he suffered a possible hairline fracture of his left wrist. He should recover completely.
Employee was cutting rubber material with utility knife and cut leg resulting in stitches.

While assisiting the rail load-out operator to close frozen doors on the bottom of DVP open hopper cars, EE's middle finger on right hand was pinched on the epad portion at the end. He was wearing all the proper PPE for the job and weather conditions.
Employee complained of back pain he said was caused by driving a haul truck. He noted that he had been having back pain for about a month, and was under the care of a doctor for his back. Also he thought at first his pain may be caused by his mattress. WC Denied claim per state law.

Injured employee was cutting a slot in a conveyor belt to be used as a side belt with a utility knife when the knife slipped and cut his wrist. The laceration required stitches. The injury did not result in lost time or restricted dury.

Injured employee was standing on 3rd rung of a portable ladder when it slipped out from under him. A vertebra was fractured when he fell to the work platform.
The injured employee was installing screw back into dust pump housing when he experience a strain while twisting and pushing. He was given a 15 pound lifting restriction by the doctor, which would not allow him to perform all of his duties as a maintenance mechanic.
Injured employee stepped on edge of crack in concrete pad near crushing plant main screen. He twisted his ankle and fell, causing a sprain.

Employee alleges that his left knee "popped" when he rose from a kneeling position. Specific Dr. diagnosis is not available at this time. Lost time from job started on 11/07/05.
Injured employee was attempting to install v-belts on a bucket elevator drive by "rolling the belts on" when the elevator ran backwards pinching a finger on his left hand. There was a laceration that required sutures.
The injured employee wass loosening hand brake on rail car when he slipped & hit bottom on the car drawhead. The accident was determined to be "lost time" on 07/11/2005.

Injured employee was standing on a 17" high by 14" wide concrete curb observing a truck being loaded. He fell while stepping back to the floor of the building, landing on his lower back.
EE ATTEMPTED TO GAIN ACCESS TO THE HAND WHEEL BRAKE ADJUSTER ON RAILCAR #LXOH 66 WHILE THE CAR WAS ROLLING DOWNHILL TOWARDS FRONT END LOADER #L8, WHICH WAS BEING OPERATED BY ANOTHER EE. EE WA S PINCHED BETWEEN THE LOADER BUCKET AND THE CORNER OF THE RAILCAR.

EMPLOYEE WAS USING A HAND HELD AIR HAMMER TO REMOVE BACKING MATERIAL FROM AROUND ANCHOR BOLTS ON CONE CRUSHER BOWL ASSEMBLY. A PIECE OF MATERIALCAME UNDER GLASSES AND GOT IN EYE.
EMPLOYEE WAS HOLDING A PIECE OF STEEL IN PLACE WHILE ANOTHER MECHANIC WAS USING A MIG WELDER ON IT. HE DID NOT REALIZE HE HAD BEEN EXPOSED UNTIL SEVERAL HOURS LATER. HE HAD RECEIVED A WELDING FLASH BURN.

EMPLOYEE WAS MA9KING BAGS ON PALLETS AND STEPPED THROUGH HOLE IN FLATBED HE FELL AND HIT HIP AND SHOULDER CAUSING BRUISED HIP AND PULLED LIGAMENTS AND MUSCLES IN SHOULDER.
HE WAS BURNING 2OLE IN METAL AND SPARKS FLEW AND BURNED HOSE CAUSING HOSE TO BURN THE PRESSURE FROM THE HOSE BLEW FIRE ON HIS GLOVE AND CAUGHT IT ON FIRE
