WALKING FROM ONE PART OF PLANT TO ANOTHER. SLIPPED AND FELL ATTEMPTED TO BREAK FALL WITH R ARM. IN DOING SO HE JAMMED R SHOULDER. INJURED SAW DR ON DAY OF ACCIDENT AND WAS ALLOWED TO RETURN TO WORK IMMEDIATELY. PER INSTRUCTIONS HE VISITED DR ON 6-25 AT WHICH TIME AN OPERATION WAS PRESCRIBED TO REPAIR TORN MUSCLES. DISABILITY BEGAN 6-26
