Marshyhope Operations LLC · Struck by falling object

The dredge operator was attempting to change an isolator block on the cylinder for the right spud. The strap broke causing the cylinder to fall on them. EE was pinned until the person who was with them picked up the cylinder and freed them. The strap which was used was not rated for this application.

Marshyhope Sand and Gravel LLC · Struck against stationary object

While attempting to tow pickup truck up elevated roadway with excavator, roadway gave way sending both operator of pickup truck and excavator into dredge pond. Pickup operator landed on ramp next to pond. Excavator operator went into pond.
